This form is to be used for any loaner technology equipment requests. All technology equipment requests must be received by the Information Technology Center at least seven (7) business days prior to the date of the event or the equipment my not be available for the event. All request are handled on a first come first serve basics in the order they are received through this system, so filling out this form does not guarantee all or any of the equipment will be available for your event.
If for any reason an event is cancelled and/or postponed and the Technology equipment is not needed, Please notify the Information Technology Center as soon as you can via email to ITC@VUU.edu
Should you need additional equipment after submitting this original request, please fill a second request form no later than seven (7) business days prior to the event.
The requester must bring their VUU employee ID card to the IT center located in Ellison Hall room 116 to pick up and sign for the equipment the business day before or the business day morning of the event and return the equipment to the IT center no later than the next business day after the event. (unless prior arrangements are made) The requester is responsible for the replacement cost of the equipment should it get damaged, lost, or stolen and will be billed to your department budget automatically.
